Group health plan renewal vs ICHRA worksheet comparison for employers
TL;DR
  • In a hypothetical 42-employee services firm in Miami with a $1,020/ee/mo group plan renewal, ICHRA at $650/ee/mo saves ~$186,000/yearbefore admin fees.
  • Group plans carry hidden costs ICHRA doesn't: broker commission (5-6%) and participation requirements. COBRA is not one of them: an ICHRA at a company with 20 or more employees carries a COBRA obligation too.
  • ICHRA has hidden costs too: employee confusion support, higher turnover risk if allowance is low, and APTC-loss friction for subsidized employees.

ICHRA vs group plan: the short answerICHRA vs plan grupal: la respuesta corta

With a group plan, the employer picks the coverage and pays a share of one carrier's premium, which the carrier resets at every renewal. With an ICHRA, the employer sets a tax-free monthly allowance and each employee buys their own individual plan. A group plan tends to fit a small team that wants one familiar network. An ICHRA tends to fit employers facing rising renewals, teams spread across states, or owners who want a fixed budget they control. To price the ICHRA side, the ICHRA administration cost calculator estimates platform fees and the ICHRA affordability calculator gives the minimum allowance per employee.

Why this comparison is always crookedPor qué esta comparación siempre está sesgada

Every ICHRA vendor publishes a comparison that concludes ICHRA wins. Every group-plan broker publishes one that concludes the group plan wins. Both leave out the numbers that don't serve them. We're going to include them.

Let's use a hypothetical but realistic 2026 example. The figures below are illustrative, not a client case. Services company (agency, consulting, or professional firm). 42 W-2 employees, mixed ages, mixed states (Miami-based but 8 remote in TX, NY, CA). Current group plan renewal: $1,020 per employee per month, up from $910 last year. Employer currently pays 70% of employee premium and 0% of dependents.

The real cost of staying on groupEl costo real de seguir en grupal

Line item Monthly Annual
Employer share, 42 employees × $1,020 × 70%$29,988$359,856
Broker commission (~5.5% of total premium)$2,356$28,278
COBRA administration (estimated)$300$3,600
Remote-employee coverage gaps (estimated replacement/leakage)$400$4,800
Total employer cost$33,044$396,534

Broker commission is almost always invisible, it's rolled into the premium, so employers don't see a "commission" line. But on a $500K+ premium, 5-6% is real money that never appears on your P&L as a separate item.

The real cost of switching to ICHRAEl costo real de cambiar a ICHRA

Now the same firm on ICHRA, offering $650 per employee per month (enough to meet the 9.96% affordability threshold for their income range in Miami-Dade zip codes):

Line item Monthly Annual
Employer reimbursement, 42 × $650$27,300$327,600
ICHRA administration ($45 PEPM average)$1,890$22,680
Agent/broker fee for employee enrollment support (optional)$420$5,040
Internal HR time for class design + ongoing support (estimated)$300$3,600
Total employer costCosto total empleador$29,910$358,920

Difference: ~$37,614/year saved. That's conservative, it assumes a flat $650 across all employees. With class design (different allowances by full-time/part-time/geography), the savings grow further.

  • Employee confusion is real. First-year ICHRA rollouts generate 3-5x the HR support tickets of group-plan rollouts. Budget 20-30 hours of HR time for the first open enrollment.
  • APTC loss hurts your lowest-paid employees. If an employee was getting a $400/mo Marketplace subsidy, and your ICHRA is "affordable" by the 9.96% test, they lose the subsidy. Their net position may be worse. Higher-paid employees don't see this problem.
  • Rural counties can break ICHRA. If your employee lives in a county with 0-1 Marketplace carriers, ICHRA may not give them a viable plan to buy. Check the HealthSherpa plan count per ZIP before committing.

  • Your renewals will keep compounding. 8-12% annual increases are the new normal. You're signing up for a cost that doubles every 7 years.
  • Group doesn't scale to remote workforces. Multi-state employees force you into either (a) paying for a national PPO with a huge premium, or (b) leaving remote employees with inferior networks.
  • Grupal no escala a plantillas remotas.
  • Participation requirements can tank you. Most group plans require 70-75% employee participation. Lose a few enrollments and the plan may not renew at all.

Use this in the order listed. Stop at the first "no":

Úsalo en el orden listado. Detente en el primer "no":

  1. Do at least 80% of your employees live in counties with 3+ Marketplace carriers? If no → ICHRA may fail for a meaningful chunk of your team. Stay group.
  3. Is your group renewal up 8%+ year-over-year? If no and you value simplicity, stay group. If yes, keep going.
  4. ¿Tu renovación grupal subió 8%+ interanual? Si no y valoras simplicidad, quédate grupal. Si sí, sigue.
  5. Do you have any 1099 contractors or mixed-class workforce you want to extend benefits to? Group can't. ICHRA can. Advantage ICHRA.
  6. ¿Tienes 1099s o plantilla mixta que quieras beneficiar? Grupal no puede. ICHRA sí. Ventaja ICHRA.
  7. Can you commit to 20-30 hours of HR time for the first open enrollment? If no, stay group for one more year and use that year to prepare.
